The Integrated Care Academy in collaboration with 5 countries (Switzerland; Ireland; Belgium; Germany, The Netherlands) is developing an Avatar, DoctorMe, to reduce social and digital isolation by supporting community-based older heart failure patients residing in rural areas of Suffolk. Based on clinical guidelines and through artificial intelligence (AI), DoctorMe will dramatically shift provision of care from professionals to patients and empower patients to become optimal heart failure self-managers through education, monitoring and therapy and medication guidance. The aim of the Passion-HF concept is to offer patients therapy that is personally tailored to their needs, which they can easily implement themselves. In this way, DoctorMe follows the principle of moving away from standardized therapy for everyone - to ensure doctor visits only when it is really necessary. Patients and their families can use the app to regularly record up-to-date data about their condition such as blood pressure, medications, and daily health status. Through regular use, and AI, the system learns and becomes more informed. Patients and carers have the opportunity to learn more about their condition, which is made ‘fun’ thanks to the app’s advanced technologies. Of significance, this world leading research brings care closer to patients and makes specialist heart failure care available 24/7.
